Cardigan Integrated Care Centre
The Integrated Care Centre will provide patients across Carmarthenshire, Ceredigion and Pembrokeshire with modern, integrated health and social care services in high quality, fit-for-purpose premises.
The new facility will incorporate a GP practice, community pharmacy and dental services, ambulatory care, reablement and rehabilitation services as well as minor injury, diagnostic and therapy services.
It will provide the opportunity for collaboration and sharing knowledge between on-site professionals, partner agencies and the wider community.
